Note: Collection of travel document may be delegated to an immediate relative provided there is a letter from the applicant authorizing his or her relative to receive the travel document. Applicants should come appropriately attired.

Since September 2008, the Consular Section has been issuing Machine Readable Passports.

What is different about the new passport?

  • It is now MACHINE READABLE at immigration entry points.
  • The color is maroon.
  • The ID photos required must now have a ROYAL BLUE background.
  • It takes about TWO MONTHS (60 days) to process because processing is done in Manila

What are the advantages of the new passports?

  • Being machine readable at immigration entry points, it will FACILITATE ENTRY because it is easier to confirm the identity of the passport holder.
  • It PREVENTS IDENTITY THEFT.
  • The cost is €50.00.

If your green passport has not yet expired, it is still valid. However, the philippine embassy strongly recommends that you renew your passport 6 months before the expiration date and the dates you plan to travel. For more information, please call 02/340.33.73 or 74.

A. PASSPORT REQUIREMENTS FOR FIRST-TIME APPLICANTS:

  • Personal Appearance is required.
  • Duly accomplished application form. Click here to download passport application form .
  • Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar duly authenticated by NSO. One photocopy of this should also be provided.
  • Three (3) colored photos of the applicant taken against a royal blue background. Applicant should be in decent attire with collar. Photo must be of good quality, and must have been taken within the last six (6) months. Photo size: 4.5 cm x 3.5 cm. Facial image size: Not less than 3 cm. DFA has the right to reject photos that do not comply with specifications and international standards. For the guidelines on the new photo requirements, click here
  • Proof of identity / Supporting documents indicating full name, date and place of birth and citizenship. One photocopy of each should also be provided.

Additional Requirements:

1. For married women, who want to use surname of their spouse:

  • Marriage Contract (MC) in Security Paper issued by NSO or Certified True Copy issued by the Local Civil Registrar duly authenticated by NSO and a photocopy of this.
  • If married in Belgium and Luxembourg, Marriage Certificate (international version) authenticated by Belgian/Luxembourgish Foreign Ministry.
  • If married in other countries, marriage contract duly authenticated by the Philippine Embassy/Consulate which has jurisdiction over area.

2. For married women who would like to revert back to their maiden name

a. If widowed: Duly authenticated Death Certificate of the deceased husband and a photocopy of this.

b. If marriage is annulled: Certified true copy and photocopy of the first page and the dispositive portion of the judgment on annulment and NSO-issued MC with the annotation on the annulment decree. One photocopy of these should be provided.

c. If divorced: Certified true copy and photocopy of the first page and the dispositive portion of the judgment on divorce secured by foreign spouse authenticated by the Philippine Embassy or Consulate where the divorce is obtained. One photocopy of these should be provided.

3. For Minors (below 18 years old)

a. Personal appearance of either parent (if minor is of legitimate status).

b. Personal appearance of mother (if minor is of illegitimate status)

c. If both parents are abroad:

c.1) Affidavit of support and consent (must be authenticated by the nearest Philippine Embassy/Consulate. Click here for the format of affidavit of support and consent.

c.2) Special Power of Attorney (must be authenticated by the nearest Philippine Embassy or Consulate General if not executed before a Consul designating the representative by name and authorizing him to apply for a passport on behalf of the minor). c.3) Passport of the duly authorized person/traveling   companion of the minor and a photocopy of this.

c.4) Identification Card of the duly authorized person/traveling companion and a photocopy of it.

4. For Muslim applicants

a. For those whose births were registered:

  • Please refer to requirements for first time passport applicants.

b. For those whose births were not registered:

b.1) Late registered Birth Certificate (BC) in Security Paper (SECPA)  issued by the National Statistics Office (NSO) and a photocopy of this.

b.2) Original and photocopy of Voter's Affidavit or other supporting documents indicating date and place of birth and citizenship, identity and tribal affiliation.

b.3) Certificate from the Office on Muslim Affairs (OMA) and a photocopy of this.

c. For converts:

c.1) Submit annotated Birth Certificate (BC) in Security Paper  (SECPA) and a photocopy of this.

c.2) Court Order granting change of name and a photocopy of this.

c.3) Shari'ah Court Order and a photocopy of this.

c.4) OMA Certificate of Conversion and a photocopy of this.

c.5) Additional supporting documents, where  necessary.

B. REQUIREMENTS FOR RENEWAL OF PASSPORT:

Additional Requirements:

1. Brown Passport or issued prior to 01 May 1995

a. Passport and photocopy of passport pages 1,2,3 (amendment). The pages showing latest Bureau of Immigration departure and arrival stamps.

b. Supporting documents with complete middle name.

2. Green Passport or issued after 01 May 1995

a.  Present passport and photocopy of inside and back cover.

b.  Photocopy of the pages showing latest Bureau of Immigration departure and arrival stamps.

3. For married women, who want to use surname of their spouse:

a. Marriage Contract (MC) in Security Paper issued by NSO or CTC issued by the Local Civil Registrar duly authenticated by NSO and a photocopy of this.

b. If married in Belgium and Luxembourg, Marriage Certificate (international version) authenticated by Belgian/Luxembourgish Foreign Ministry.

c. If married in other countries, marriage contract duly authenticated by the Philippine Embassy/Consulate which has jurisdiction over area.

4. For Minors (below 18 years old):

a. For illegitimate child, personal appearance of either parent is required.

b. If minor is NOT traveling with either parent:

b.1) Original and photocopy of DSWD Clearance

b.2) Affidavit of Support and Consent Click here for the format of affidavit of support and consent

c.) If both parents are abroad:

c.1) Affidavit of support and consent (must be authenticated by the nearest Philippine Embassy/Consulate. Click here for the format of affidavit of support and consent

c.2) Special Power of Attorney (must be authenticated by the nearest Philippine Embassy/Consulate. Click here for the format of special power of attorney

c.3) Passport of person traveling with the minor and a photocopy of this.

5. For the Replacement of Lost Passports

6. Passport Fees:

Processing €50.00 (processing time may take approximately 60 days)

Replacement of Lost valid Passport  €90.00 + affidavit of Loss  €25.00

7. List of Supporting Documents:

  • Marriage Contract
  • Voter's Registration Record
  • PRC / IBP ID
  • Baptismal Certificate
  • Land Title
  • Seaman's Book
  • Driver's License
  • Income Tax Return (Old)
  • Government Service Record
  • Old SSS-E1 form/or digitized SSS-ID
  • School Form 137 or Transcript of Records
  • Other documents that show full name and birth details of applicant and citizenship

C. REQUIREMENTS FOR THE ISSUANCE OF A TRAVEL DOCUMENT

A Filipino citizen who does not have a valid passport, for one reason or another, and wishes to return to the Philippines may apply for a travel document. This document is valid for one direct flight to the Philippines.

The following documents are required:

1. Duly accomplished Application Form for the issuance of a Travel Document;

2. Photocopy of old Philippine passport or NSO-certified Birth Certificate;

3. Travel Document fee (€30.00);

4. Three (3) identical passport photos size 4.5 cm x 3.5.cm, in color, with  plain, white background:

  • Applicant must be wearing appropriate attire with collar and sleeves,
  • Applicant may wear eyeglasses provided photo shows a clear view of applicant's eyes and face.
