Requirements for report of birth of a Filipino born overseas

(To be registered within 30 days for the date of birth of the child with the Philippine Embassy having Consular jurisdiction over the place where the child was born)

Bring Originals and Four (4) sets of Photocopies

1. Duly accomplished form for Report of Birth (FA Form No. 40) click here to download form
2. Authenticated birth certificate of the child (international version)
3. NSO authenticated Marriage Certificate of the parents / Foreign Marriage Contract
4. Photocopies of parents' valid passports
5. Four (4) pieces of passport size photos of the child
6. Fees: Report of Birth          € 25.00

Additional requirements for delayed registration of report of birth

1. Joint Affidavit for delayed registration signed by parents  ---        € 25.00

NOTE: (for cases of delayed registration of report of birth) if both parents are already foreign nationals upon report of birth, the former Filipino parents must present the Certificate of his/her Naturalization.
