LIST OF REQUIREMENTS

FOR

NON-QUOTA IMMIGRANT 13(a) APPLICANTS

(Spouse and Dependents)

2. Authenticated Certificate of Birth
3. Certificate of Good Moral Conduct
4. Certificate of Nationality
5. Medical Examination Form (FA Form No. 11) - (duly filled-out and signed by a General Medical Practitioner click here to download FA Form No. 11
6. Laboratory Results and Analysis duly signed and attested by the General Medical Practitioner (Note:  every page of the medical examination results should bear the stamp and signature of the authorized medical practitioner)
7. Certification from the doctor for medical clearance against AIDS Virus
8. Affidavit executed by the Filipino spouse indicating the following: a) he/she is married to the applicant; b) reasons for migration; c) other personal details contained in the Philippine passport, etc.
9. Authenticated copy of marriage contract (if married in the Philippines, document should have been authenticated by the National Statistics Office and the Department of Foreign Affairs in Manila.)
10. Photocopy of plane ticket or a confirmed booking from a travel agency
11. Photocopy of applicants Belgian/Luxembourgish Identification card
12. Photocopy of Filipino spouse passport ( if there are children, also submit photocopy of passport - if holder of a Philippine passport).

Note:

12. 1. All required foreign documents that will be submitted to the Philippine Embassy (Consular Section) should be written in English and duly authenticated/legalized by the Ministry of Foreign Affairs of Belgium/Luxembourg.  Thus, if the documents secured by the applicants form their respective municipalities/cities are written in French, Dutch or other foreign language text, an official English translation should likewise be secured.

12. 2. Other requirements not listed on the above list might be required for submission, if warranted.

12. 3. Applicants are required to submit three (3) set of application forms (together with the original copy of all the corresponding documents required), as cited above.

*In most cases, the Foreign Ministry requires an authentication by its Justice Ministry before it authenticates a document.
